Некоторые технические проблемы при психоанализе перверсий

Мне кажется, что решающее значение для аналитика, занима­ющегося лечением пациентов с хорошо структурированными пер­версиями — на невротическом ли, или на пограничном и нарцис-сическом уровнях — имеет поддержание позиции технической нейтральности относительно того, какой будет сексуальная ориентация пациента по окончании лечения. Это не означает, что ана­литик должен сохранять пассивное безразличие к пациентам, де­монстрирующим, например, садистскую перверсию, которая может угрожать другим людям или им самим. Техническая нейт­ральность всегда включает в себя озабоченность безопасностью пациента и тех, с кем он взаимодействует, а также защиту самой ситуации лечения.

Решающее значение имеет то обстоятельство, что аналитик позволяет пациенту определить собственную сексуальную ориен­тацию и выбор объекта. То, действительно ли аналитик искренне уважает свободу пациента в определении себя гомосексуально или гетеросексуально или просто делает это на словах, может оказать решающее воздействие на характер аналитического лечения и ту меру, в которой оно способно помочь. Аналитик, возможно, дол­жен освободить себя от общепринятого предубеждения, подходя­щего для других ситуаций помимо аналитической. Свобода анали­тика переживать свои собственные полиморфные перверсныс сексуальные тенденции в эмоциональных реакциях на материал пациента, свобода идентификации с сексуальным возбуждением пациента, так же как и с объектами пациента, как часть отливов и приливов контрпереноса может помочь вынести на поверхность примитивные фантазии пациента, связанные с преэдиповыми де­терминантами полиморфной перверсной сексуальности, которые иначе останутся диссоциированными, вытесненными или даже сознательно подавленными.

Пациенты с организованными перверсиями часто обнаружива­ют фальшивую инкорпорацию или псевдоинкорпорацию интерпре­таций аналитика как часть крайне незаметного обесценивания эди­пова отца, проецируемого на аналитика (Chasseguet-Sinirgel, 1984), создавая, таким образом, особенные трудности для лечения.'Па­циент ведет себя так, "как будто" инкорпорирует интерпретацию, бессознательно насмехаясь над аналитическим процессом. Это является олицетворением бессознательной насмешки над властью эдипова отца и псевдоидентификацией с ним, основанной на анальной регрессии и деструктивной порче того, что получается.

Такое развитие может принять форму задавания пациентом про­ясняющих вопросов, указывающих на то, что он слышал интер­претации аналитика, "но не совсем" и пытается незаметно изме­нить их для лучшего понимания. Как будто пациент "вот-вот" что-то поймет; у аналитика часто остается смутное ощущение бес­покойства о том, идет ли настоящая работа. Тот факт, что в та­ких обстоятельствах могут возникать глубокие трансферентные рег­рессии и происходит значительное изменение трансферентнои парадигмы, убеждает аналитика в подлинном характере взаимодей­ствия, хотя сомнения по поводу того, что является реальным, а что "как бы", остаются.

Эту защитную констелляцию следует отличать от бессознатель­ной порчи интерпретаций и их жадной инкорпорации и деструк­ции, типичных для нарциссических личностей. Действительно, осознание аналитиком того факта, что этот процесс происходит также при организованных перверсиях у ненарциссических паци­ентов, может сделать его чувствительным к этой особой защите и позволит ему ее интерпретировать. Защита может принять форму ребяческого хвастовства пациента тем, чему он научился в анали­зе, или обсуждения своего анализа с другими людьми при неза­метном высмеивании эдипова отца — символически заимствуя его пенис и унижая его [пенис] в этом процессе.

Важно внимательно исследовать безразличие или отталкивание пациента по отношению к женским гениталиям. Вытеснение сек­суального возбуждения, лежащее в основе такого обесценивания, следует дифференцировать от неспособности к эротизации повер­хностей тела в целом, характерной для некоторых тяжко больных пограничных или нарциссических пациентов. В последнем случае нельзя ожидать никаких изменений, до того как произойдет раз­витие регрессии в переносе, позволяющее реактивировать то, что относится к очень ранним отношениям матери и ребенка, и при­водящее к развитию новой способности к нежности, вновь заря­жающей кожную эротику, идеализации частей тела и ранним кор­ням полиморфной перверсной сексуальности в целом. Напротив. у лучше функционирующих пациентов, у которых неприязнь и отвращение к женскому телу являются регрессивной защитой про­тив чрезмерной кастрационной тревоги, проработка этой тревоги, страха и отталкивания по отношению к могущественному и жес­токому отцу и заторможенной способности к идентификации с ним обычно предшествует способности выдерживать восстановление сексуального возбуждения от женских половых органов.

При перверсиях, так же как и при любых других случаях с тя­желой сексуальной заторможенностью, сексуальные фантазии па­циентов (как и мастурбаторная активность и действительное сексуальное взаимодействие) должны быть тщательно исследованы. Часто незаметное защитное избегание такого исследования прояв­ляется в том, что пациент готов отрыто обсуждать некоторые ас­пекты своего сексуального опыта при тщательном избегании дру­гих аспектов; иногда аналитику требуется много месяцев, чтобы узнать об этих избегаемых областях.

В других случаях пациент щедро раскрывает свои хаотические сексуальные фантазии и поведение — при явной полной "свобо­де" сексуального выражения — защитно избегая центральных аспек­тов переноса, особенно примитивных типов негативной трансфе­рентнои диспозиции. Хаотическая сексуальная жизнь пациента, характеризующая все его взаимодействия, используется как защита от глубоких объектных отношений с аналитиком, и задача стано­вится совершенно иной: анализ объектных отношений в переносе должен быть вынесен на передний план и связан с защитным "из-мазыванием" аналитической ситуации примитивным сексуальным материалом.
