ОСНОВНОЙ УРОК 1.

A DAY IN THE LIFE OF A GP

Vocabulary: Один день из жизни врача общей практики

Grammar: Настоящее простое время, настоящее длительное время, прошедшее простое время

1. READING AND VOCABULARY

A Day in the Life of a GP

My name is Alison Thompson and I am a GP. It means I am a general practitioner. I don’t work in a hospital; I work in a surgery. A surgery is a medical center for the local community. If you’re a new patient living within our catchment area, you can register with our surgery.

I work with five other doctors, three nurses, two lab technicians, a physiotherapist, a pharmacist, a health visitor and a receptionist. We organize shifts of a locum GP if any of the doctors is absent. On Saturday mornings we work on a rota.

When our patients have dental problems, they contact their dentists in normal working hours. If the problem is urgent, they call the emergency contact number. There is a rota of dentists who deal with emergencies out of working hours.

I always wake up quite early -- at 6 in the morning, because I’m on call at 7. To be on call means to take emergency calls from patients. Some mornings I have no calls, but usually I speak to one or two people about a problem.

I arrive at the surgery at 8: 45 and start work at 9. Lots of people usually want to see me and wait in the reception area. Some people have a minor problem like a sore throat, a runny nose, a rash, a slight fever or headache; others can’t sleep at night or feel nauseated. Sometimes I deal with more serious cases (e. g. diabetes) and refer my patients to specialists. Every day is different.

My surgery hours are from 9 to 11: 30. But I always work until 12 non-stop. I have no coffee break, and sometimes no lunch, either. I always have a lot of paperwork: I write referral letters, send and receive messages, file and scan documents into patient records. I usually do this at lunchtime.

In the afternoon, we have another two hours to see patients. It is the time when we give vaccinations, e.g. for influenza, and perform minor surgery such as removal of warts and moles.

After that, I may have a meeting with the other staff. Sometimes I go to other surgeries for meetings or have home visits.

I have some problems, of course. I don’t like all that paperwork. And I don’t like patients who are rude or aggressive to me or the other staff in the office.

I get home at around 6 or 7 o’clock in the evening. I always take paperwork home. I have 15 or 20 blood and urine test results, X-ray and MRI reports every day and I check them at home. Sometimes I’m on call again or visit more patients.

I have no problems with sleeping. In the evening I feel so tired that I fall asleep immediately.

What is the nurse doing?

TEST

The work of a GP.

My typical day at Balham Park Surgery usually starts at about 8.15 am when I deal with letters and other paperwork until my first patient arrives at 8.40 am. I then see about 15 patients individually until 11.40 am. We then have a 20-minute meeting with everyone who works in the surgery, including the reception and nursing staff. We review difficult cases, allocate home visits and discuss patients who are coming in for a same day emergency appointment.

My day continues with more paperwork, such as writing referrals to hospital specialists and reviewing the results of blood and other tests we have sent to the pathology lab. Lunch is usually a sandwich while I carry on with admin. Once a week we try to have a team lunch with all the surgery staff.

After that I do a home visit for patients who are too frail or elderly to come into the surgery. The afternoon continues with tackling my phone call list before my next clinic starts, which runs from 3.00 pm until 5.40 pm. I usually finish work at around 7 pm, after finishing off emails, phone calls and letters.

One morning a week I work at a local residential home for the elderly, where many of the patients have dementia. I work closely with an advanced nurse practitioner, particularly in the context of trying to provide very high-quality end-of-life care.

One afternoon a week during term-time is spent teaching medical students from St Georges Hospital about Primary Care. I also participate in the extended hours rota, which involves working on Saturday mornings about once every four months.

TEST

Переведите текст.

Every medical office has a waiting room, and this room gives a patient their first impression of a practice. Chairs and tables and maybe a few plants and magazine racks may not sound so important, but the decor of the waiting room can do a lot to set a patient at ease during an anxious time.

The next thing a patient sees is likely to be the exam room, which also needs to appear professional yet soothing. Exam room furnishings include an exam table, a chair or stool for the doctor, often on wheels, and usually a chair for the patient's companion. There is also a counter and cabinets for supplies. Depending on the practice, there may also be a computer and large-screen monitor for a doctor to show the results of any x-rays or scans to the patient. Also medical offices require specialized filing and storage solutions for patient documents reports.

Most medical offices require some basic diagnostic tools. These can include thermometers, pulse oximeters, sphygmomanometers (better known as blood pressure monitors), audiometers, and stethoscopes. Larger equipment can include EKG machines.

Behind-the-scenes diagnostic equipment often includes microscopes and slide preparation supplies for sample analysis. More complex testing usually occurs at an offsite laboratory.

Modern offices prefer single-use tools for a variety of purposes as they are more convenient and sterile. But some items such as scalpels, speculums, and extractors need to be cleaned and sterilized for reuse. This procedure involves the use of an autoclave. Consumables include any supplies that need to be replaced regularly, such as sterile gloves, paper exam gowns and covers for exam tables, cotton swabs, gauze, tongue depressors, chemical test strips, suturing equipment, syringes, disposable instruments, and of course all the standard office, kitchen, and restroom supplies.

2. Сформулируйте вопросы, начинающиеся с данных вопросительных слов и соответствующие приведенным ответам.

Where …….? -- More complex testing usually occurs at an offsite laboratory.

Why…..? -- Modern offices prefer single-use tools as they are more convenient and sterile.

What…….? -- They include any supplies that need to be replaced regularly.

What….? -- There is an exam table, a chair or stool for the doctor and a chair for the patient in the exam room.

Where…? -- Scalpels, speculums, and extractors are cleaned and sterilized in an autoclave.

3. Опишите кабинет врача (терапевта, педиатра, стоматолога), используя актуальную грамматику и лексику урока.

READING AND VOCABULARY

The Human Body

Inside the head is the brain, which is responsible for thinking. The top of a person's scalp is covered with hair. Beneath the hairline at the front of the face is the forehead. Underneath the forehead are the eyes for seeing, the nose for smelling, and the mouth for eating. On the outside of the mouth are the lips, and on the inside of the mouth are the teeth for biting and the tongue for tasting. We swallow food down the throat. At the sides of the face are the cheeks and at the sides of the head are the ears for hearing. At the bottom of a person's face is the chin. The jaw is on the inside of the cheeks and chin. The neck is what attaches the head to the upper body.

At the top and front of the upper body, just below the neck is the collar bone. On the front side of the upper body is the chest, which in women includes the breasts. Babies suck on the nipples of their mother's breasts. Beneath the ribcage are the stomach and the waist. The navel, more commonly referred to as the belly button, is located here as well. On the inside of the upper body are the heart for pumping blood and the lungs for breathing. The rear side of the upper body is called the back, inside which the spine connects the upper body to the lower body.

The arms are attached to the shoulders. Beneath this area is called the armpit or underarm. The upper arms have the muscles known as triceps and biceps. The joint halfway down the arm is called the elbow. Between the elbow and the next joint, the wrist, is the forearm. Below the wrist is the hand with four fingers and one thumb. Beside the thumb is the index finger. Beside the index finger is the middle finger, followed by the ring finger and the little finger. At the ends of the fingers are fingernails.

Below the waist, on left and right, are the hips. Between the hips are the reproductive organs. At the back of the lower body are the buttocks for sitting on. They are also commonly referred to as the rear end or the bum (especially with children). The internal organs in the lower body include the intestines for digesting food, the bladder for holding liquid waste (urine), as well as the liver and the kidneys. This area also contains the woman's uterus, which holds a baby when a woman is pregnant.

The top of the leg is called the thigh, and the joint in the middle of the leg is the knee. The front of the lower leg is the shin and the back of the lower leg is the calf. The ankle connects the foot to the leg. Each foot has five toes. The smallest toe is often called the little toe while the large one is called the big toe. At the ends of the toes are toenails.

Составьте рассказ о том, как будет проходить осмотр. Когда точно известно, что медицинский персонал собирается сделать или есть уверенность, что это действие произойдет, используется конструкция « to be going to ».

The nurse

- take vital signs: blood pressure, height, weight, heart rate, respiration, and body temperature.

The lab technician

-  conduct a series of tests to assess the condition of the heart, lungs, circulatory system, and auditory system.

The doctor

- review medical history

- check the appearance of the skin and look for signs of vitamin or mineral deficiencies or sleep deprivation, memory problems, bruising or bleeding problems

- examine the heart rate and listen for any irregularities

- examine the lungs in order to determine any respiratory issues

- test extremities for strength and blood flow.

READING AND VOCABULARY

Taking complaints toothache   Do you feel any pain? Which tooth troubles you? Does this tooth hurt? You should visit a dentist more often. We should have a look at the fillings.

During a dental exam, your dentist or hygienist will ask about any health problems you have or medications you're taking and discuss how they might affect your oral health. If you have diabetes, for example, you're at increased risk of gum disease. Any medication that contributes to dry mouth can increase your risk of tooth decay.

Now that you have shared your health concerns, they can start working inside your mouth. The first thing they will do is give your teeth a thorough cleaning. They start by scraping off built up plaque and tartar that collects above and below the gum line.

Now that your pearly whites are squeaky clean, it’s time to have a look at any problem areas in your mouth. A metal probe with a small angled mirror will be used, which will help them see behind and between teeth and gums, as well as check for the softening of tooth enamel and dentin. Your dental hygienist explores every surface of every tooth to uncover new cavities and examine the quality of existing fillings. Over time, fillings can break down and require replacement.

They will also be on the lookout for the swelling of gums in any areas, mouth sores, and redness.

After the dental exam is complete, your dentist will let you know exactly what is going on in your mouth and will advise you about next steps. Sometimes this involves scheduling another dental appointment for a procedure while other times your dentist will give you advice about what you can do at home to better your oral health.
